Your First Visit

Your initial exam will take approximately one hour. This visit will begin with record taking and recording, then proceed with a comprehensive evaluation of your oral and dental health and a propose treatment plan suitable for you. 
(Please be advised that depending upon your dental needs, you may or may not have your teeth “cleaned” at your first appointment, please let us know ahead of time of your preference)

Records Release from another office

Please fill out the Records Release Form electronically and email to us so that we can request records from your previous office on your behalf.

X-RAYS

Digital x-rays are used to minimize radiation exposure, they produce higher quality images and since there isn't any developing time--digital radiography can also shorten your dental appointment! 

PHOTOS

High quality Intra-oral and Extra-oral photos are taken to assist diagnosis and treatment recommendation. They also serve as great educational tools for patients as well as for insurance pre-authorization and documentation purposes.

TREATMENT SEQUENCE

Dr. Chan will put careful consideration to YOUR customized treatment plan and prioritize the conditions that need immediate attention first while also focusing on preventative treatment so you can avoid tooth or oral health conditions in the future.

FINANCIAL AND INSURANCE

At our office we accept cash, checks, and most credit cards for payment.
We accept most major insurance plans and are a Delta Delta  PPO and Premier provider. As a courtesy we will do the billing to your insurance company and do our best to minimize your out of pocket expense.
